This document must be completed and returned to Sims Family Tours & Travel before any selling or booking can begin.

Credit Card Authorization Form

This form must be sent in to authorize any Credit Card Transactions NOT made directly to a supplier.

Booking Form

This booking form MUST be sent in for any booking in which you expect to receive a commission once the booking is made. HINT - Save a copy and put in your information, then fill out and send in the variables for each sale.
